Safety of GH replacement therapy in patients with non-functioning pituitary adenomas

The risk of tumor enlargement or recurrence is an important factor to consider when administering GH replacement therapy (GHRT) to hypopituitary patients with a history of pituitary adenoma. In this case–control study, Olsson et al studied the rate of tumour progression in 121 patients with non-functioning pituitary adenomas (NFPA) receiving long-term GHRT, for an average duration of 10±4 years.

It was found that in patients with a known residual adenoma, 63% had no detectable tumour enlargement. Ninety percent of patients who had no visible residual adenoma did not have a recurrence. The study found that ten-year survival rates were 74% in patients treated with GHRT, compared to 70% in controls. There have only been a few similar studies in the past and this one, by far the largest, conducted over a long period of time, provides support that long-term use of GH replacement in hypopituitarism can be considered safe in patients with NFPA.
